INKPARK CONSENT TO APPLICATION OF TATTOO

I acknowledge by filling this agreement that, I have been given the full opportunity to ask any questions which I might have about the obtaining of the tattoo from InkPark Tattoo Studio and that all of my questions have been answered to my full an total satisfaction, I specifically acknowledge that I have been advised of the facts and matters set from below, and I agree as follows.

1) I confirm that I do not have any blood-related or communicable diseases and I am not under the influence of drugs or alcohol.

2) I understand that allergic reactions to tattoo pigments or procedures are possible and I accept this risk.

3) I acknowledge the risk of infection and agree to follow proper aftercare instructions; any touch-up due to my negligence will be at my own expense.

4) I understand that the final tattoo’s color and design may vary, and colors may appear less bright on darker skin tones.

5) I understand that a tattoo is permanent and no guarantees have been made regarding its removal or alteration; I have no condition affecting my decision.

6) I confirm that I am over 18 years of age and that all provided information is true.

7) I voluntarily consent to the tattoo procedure and any necessary actions performed by InkPark Tattoo Studio staff.

8) I agree to release and hold harmless InkPark Tattoo Studio and its employees from any claims related to the tattoo procedure.

9) I confirm that I have received and understood the aftercare instructions provided by InkPark Tattoo Studio and agree to follow them properly.

10) I give permission to InkPark Tattoo Studio to take photographs or videos of my tattoo for portfolio, website, or social media use.

11) I understand that deposits are non-refundable and the full payment must be completed as per studio policy.
